What?! This ain’t no appendix

It was late evening. A young girl of 8 years old, was crying and screaming in pain. The pain she localised it to the right lower quadrant of the abdomen. It has been three days now and the pain was getting worse. The parents took her initially to a general practitioner who then referred her to us. On examination, her pulses were racing and she was warm. Her abdomen was so tender all over but she braved herself to be examined. Her abdomen was tensed and tender. It was also guarded(stiff). She was taking short quick breaths. Having admitted her, she was quickly resuscitated and her pain was relieved. Despite this, the pain was still present. She was booked for surgery with a suspicion of a perforated appendix.

I was notified that she was going into surgery. Not long after, I was called into the operating theatre.

So what do you think is going on?

    The shape and color look like an ovary twisted and infarcted, but the ovary is markedly larger than it should be, so there might be a cyst. In a young child you would have to consider a cystic teratoma maybe even more likely than a simple cyst.

    Poor girl. I hope she recovers soon. It must have been a very traumatic experience for her. I’ve had 4 surgeries and i know how it’s like.

    Kakaka.. i just realised that srp.. made the same diagnosis…

    … but did you see the difference between a pathologist and a surgeon…
    … I jumped to a conclusion … srp made some suggestions … and left it open pending more info.

    That is actually a twisted left ovarian mass which was lying on her right side. The right ovary can be seen in the second picture. Well, I am still awaiting for the pathology report. I had two similar cases before and both were of twisted haemorrhagic ovarian cyst. None were of teratoma.

    I should have her results with me tomorrow. She is well at present but her future will always be an issue. I will update once the result is up
