This is quite straight forward. Care to guess?

Both are different views of the breast from the same patient in her early 40s presenting to us with a breast lump.

Enjoy.

Why blog?

This is a question that frequently crosses my mind. I never dwelt on it as with the passing days, time took to the better of that. However, today was different. I finished my call and tired, unable to produce an article for the blog. I remembered what Shireen used to ask me, “Why blog?” Then, I realised that it is Chinese New Year eve and I haven’t wished one of my closest friend, Bernard yet. Lately, the sms, IM, emails and the blog has evolved to a level that made it so easy to convey a message. But, I felt a loss of that human touch, the voice, the intonation, the rhythm and the depth of a human voice. So, I quickly reached for my handphone and called Bernard but sadly it was unanswered. Later, I find out his battery died from messaging him on IM. My post call hangover continued till the late evening and despite me trying to write an article, I was numb.

Hi, it is festivities again. This time it is Chinese New Year on the 18th February 2007

New Year festivities traditionally start on the first day of the month and continue until the fifteenth, when the moon is brightest. The 15th day of the new year is called the Lantern Festival, which is celebrated at night with lantern displays and children carrying lanterns in a parade.

Well, this year is the Year of the Pig, the Fire Pig to be exact. Legend has it that in ancient times, Buddha asked all the animals to meet him on Chinese New Year. Twelve came, and Buddha named a year after each one. He announced that the people born in each animal’s year would have some of that animal’s personality.

This friendly lady presented to me 2 years ago for a painful swelling which resolved after a day of admission. She then went missing and reappeared with the same complaint a few days ago. I was in Kuching then and she refused to go home. She insisted on seeing me and after seeing me she insisted on going back only after surgery was done no matter how long it was going to take. She knew that if she were to go home she would end up defaulting treatment as she was a single mother and financially strained.

She has 9 children and this swelling has been there ever since (more than 10 years), growing bigger and bigger till present size.

The swelling was so big that it caused her to have difficulty in taking care of herself. The underside of the swelling developed fungal infection due to the moist and ulcerated.

The operation :
the neck of the hernia

the sac opened and the sneak view of its contents

almost two thirds of the small bowel was within the mass(hernia)

The sac and the excess skin excised revealing the abdominal wall defect: she is lucky because if the bowels twisted within its axis, the blood supply would be obliterated and she would end up dead with two thirds of her small bowel ischaemic.

The defect was closed and repaired with a mesh. She is well and presently at home.

** see the comments for why she delayed her treatment.
